Is there a fee to receive referrals?
We will provide you with a FREE personalized list of organizations that provide fall prevention services. Some of the organizations we refer you to may have fees.
Why do you need to collect personal information?
The information collected will be used to identify the best services to meet the needs of the older adult. The more information we are given, the more personalized the referrals will be.
What do you do with the information I provide?
All information collected will be kept confidential. Your information will not be sold or shared.
When will I receive the personalized referral information?
Once we receive your Request for Referral Form, please allow up to 3 business days for us to process your request. The information you provide will be used to create a personalized list of organizations that offer the fall prevention services. You will receive a list of organizations via phone or e-mail based on the preference you indicated on the Request for Referral Form.
